Dr. Lex Kenerly at Kenerly
Orthopaedic Center in Jesup, Georgia needed to have his patients fill out a
medical questionnaire online. Before coming to T. Davis Consulting to create a
more dynamic approach, his patients had to print out a PDF form, fill out the
form by hand, and then fax it in. Now, his patients simply go to the web site,
fill out the form online, and click the submit button. Dr. Kenerly then has immediate
access to his patient’s information stored on a secure web server.

| Kenerly Orthopaedic patients can also request an appointment online. After filling out the request online, an email is sent to the doctor's office. The email contains no confidential information, only the name of the patient. Other pertinent information is stored on a secure server that only the doctor's office has access to. |

Pain Man
|
In order to help patients more clearly identify pain, Dr. Kenerly asks that they mark the location on a body diagram. The PDF diagram was simply drawn on, but doing this online seemed a little awkward. But the Pain Man solved the problem. This custom written diagram made using HTML and JavaScript, allows the patient to simply move the cursor over the body diagram and click the section of the body when it is highlighted. Three different severities of pain are identified by three colors. The diagram is stored on a secure server and redisplayed exactly as the patient identified for the Doctor to review.
